Jennifer Johnson Muskat

Regional Director

Jennifer acts as the ‘matchmaker’ of Home Care Assistance. She is able to match clients with caregivers by listening to and respecting their needs and preferences. She understands the complexities of working with families and individuals who need care. Jennifer works hard to meet their home care goals each and every day and provide high-quality care.

Jennifer has been working with older adults and Home Care Assistance for almost a decade. Clients, families and other elder care professionals consider her to be a true industry expert and leader.

Jackie Paras

Client Care Manager

Jackie graduated from San Diego State University with a degree in Communications. She has five years of professional work experience in public relations for a health and wellness company, as well as in account management for health and home care companies. Jackie excels at developing deep, meaningful relationships with her clients and is their number one advocate, always ensuring that they receive the best care. This expertise in client relations and her innate ability to empathize with others allows Jackie to thrive in the home care industry, which lets her make a difference in the lives of those who need support.

Sean Cygan

Operations Manager

Sean Cygan has over 25 years of professional experience working in the home care industry. Prior to joining Home Care Assistance, Sean worked as a Site Coordinator, Human Resources Staffing Specialist/EEO Coordinator, Human Resources Generalist and Human Resources Staffing Coordinator. Sean brings his expertise in communication and coordination to his current role as Senior Staffing Manager, where he is able to match clients with caregivers based on their unique needs and preferences. Over the course of his professional career, Sean has worked with a variety of older adults, including seniors with disabilities, and he has a passion for helping them maintain their independence with the aide of exceptional in-home care.

Rosanna Lopez

Lead Staffing Manager

Rosanna has a Bachelor's degree in Nursing and experience as a Registered Nurse in the Phillippines and a Licensed Vocational Nurse in California. She was also an Administrator for residential care facilities for the elderly and a Hospice Nurse for Hospice by the Sea in San Diego. With over ten years of experience in staffing, recruitment and case management, Rosanna's expertise is well-suited to her role as Staffing Manager. Rosanna is dedicated to her profession and loves making a difference in peoples' lives and seeing the smiles on their faces.

Mina Yap

Care Advisor

Mina is a Licensed Vocational Nurse and began her career in health care in 2011. Since then, she has learned a lot in her many roles. She started as a hospice nurse and then became a nurse for concierge doctor, home health nurse, and, finally, intake coordinator for a home health & caregiving agency. Mina believes that helping people who can't help themselves is what we are here for. By working for Home Care Assistance, she feels she can make a difference in the lives of seniors and help the home care industry thrive.

Gabrielle Pumpian

Home Care Liaison

Gabrielle has been in the health care arena in San Diego County for nearly a decade. After graduating from the University of Oregon with a Bachelor of Arts in International Relations, she decided to transition her focus into the health care industry. She learned first-hand the importance of excellent care in a home setting after the unfortunate stroke of a family member. This experience shifted her perspective and career ambitions towards helping others obtain the resources they need in order to live a dignified and happy life in their home environment. Shortly after, she began working as a Marketing and Sales Representative for a local home care agency, eventually moving onto an opportunity as the Director of Marketing for a large skilled nursing and rehab facility in San Diego County. Her heart remained in the home care world, and she is fulfilled every day supporting and working with people who are in need of caregiving services.

Jonna Hernandez

Regional Director of Bus. Dev., SoCal

Jonna's professional experience is in sales, mentoring, and strategic market development. Her education includes a B.A. in Human Communication Studies from California State University, Fullerton and a M.A. in Counseling from Azusa Pacific University. Jonna started in the health care industry in 2011 as an Admissions and Marketing Director for a skilled nursing facility. She has extensive sales experience in the post-acute sector including skilled nursing facilities, home health, and non-medical home care. Jonna's most recent position was the Area Operations and Sales Manager for a non-medical home care agency for Orange County, South Bay, and Inland Empire regions. Jonna's personal philosophy is to change the outlook of how others view and experience senior healthcare. She is absolutely thrilled to be a part of an innovative team that is truly defining how the world is aging.

Jason Herald

Home Care Liaison

Jason is a San Diego native and has been in the field of marketing and sales for 20 years. He holds a Residential Care for the Elderly Certificate (RCFE) and began his career in the health care industry 8 years ago as an administrator for an assisted living facility. From there Jason moved on to a Marketing Director for a nationwide Assisted Living Community. He understands the importance of home safety and is passionate about helping seniors age in place. Jason was fortunate and able to assist in the caretaking of his grandmother so that she was able to age in the comfort of her own home and he appreciates the value of good caregiving services.

Amy Joiner

Client Care Manager

Amy began doing Geriatric Care Management in 2008 in Hawaii and has been providing Care Management to older adults for over 10 years. She has an AS in Criminal Justice and received her BA in Social Work. Amy then went on to earn her Master's Degree in Social Work from Hawaii Pacific University, where she assisted in overseeing homeless shelters. She also supervised bachelors-level social work and psychology students from the University of Hawaii and Pacific University who were specializing in Micronesia and Native Hawaiian cultures.

Amy takes pride in creating care plans designed specifically for the needs of each individual client and their family while adding a personalized touch. She enjoys serving as a liaison between clients, their trustees, attorneys, physicians, nurses and family members, as well as educating caregivers and the community on the aging process.

Amy is a member of the Phi Alpha National Social Work Honor Society and Phi Gamma Mu Honor Society, which espouses a high level of accountability and abides by the National Association of Social Workers Code of Ethics. Amy focuses on continually learning and encourages caregivers and those in the community to do the same.

Crystal Montero

Staffing Manager

Crystal brings over 15 years of customer service experience in everything from hospitality to home care to her role with Home Care Assistance. Specifically, she has trained to work with high-end clients who are facing difficult situations. Crystal received a double bachelor's in Public Health and Gerontology with a minor in Social Work from San Diego State University. She found her passion for seniors by doing a public health internship where she worked with seniors. She fell in love with it and even stayed in college an extra year in order to obtain a double major.

Her favorite part of her job is working with the elderly and forming relationships with them. Crystal comes from a Hispanic family and feels that valuing elders is something that was instilled in her from a young age and is strongly tied to her culture. Crystal believes working with seniors is a privilege and enjoys giving back to her local community.

Aaron Smith

Home Care Liaison

Aaron has two years of experience doing operations and business development for a home care organization in Orange County. He is adept at management and operations and his passion is meeting with and educating community partners. Aaron had great success being a trusted home care resource for medical centers, skilled nursing facilities, and senior living communities.

He graduated from California State University of Long Beach with a Bachelor's Degree in Sociology and a minor in Communication Studies. As a student, he decided to get into home care because he wanted a career which would allow him to help those in need and make a difference in the world. This was further influenced by the fact that his mother was a caregiver for 16 years. She was the primary caregiver for a paralyzed woman who served as a key role model in Aaron's life. She left such a monumental impact on his life, which greatly influenced his career choice.

Aaron strongly believes that every day is an opportunity to improve, learn, and be better than the person he was yesterday. As an innovative thinker and someone striving for continuous improvement, he strongly follows the company's mission to change the way the world ages.
