Jazmine Soubra

Client Care Manager

Jazmine brings 25 years experience in healthcare. She has worked in outpatient medicine, chronic conditions management and geriatric case management. Jazmine has vast nursing and clinical background experience working at Kaiser Permanente and from working with San Francisco Department of Public Health. She earned her Bachelor's degree in Nursing with a minor in Health Education at San Francisco State University. Jazmine designed a diabetes self-management education program for seniors living with chronic conditions while she was working on her masters degree in Public Health at San Jose State University. She discovered a deep connection with the geriatric community when she taught pre-diabetes classes. Jazmine believes home care for seniors is an integral part of healthy aging. Jazmine's passion as a Registered Nurse involves engaging, educating and empowering geriatric population through a holistic approach.

Cat Tomboc

Client Care Manager

Cat is joining the Home Care Assistance team with over four years of professional experience in home care. She first experienced senior care in high school when she volunteered at a nursing care home. While helping her family care for her grandmother on hospice, she found the inspiration to become a nurse. Cat is a licensed vocational nurse and has held different roles in the senior care world including in-home care, memory care, post-acute care, assisted living and skilled nursing/rehabilitation. Cat enjoys working with the senior population and truly believes that seniors should age in place, in the comfort of their own home. Cat has a passion for helping others and she strives to make a positive difference in their lives.

Colleen has been in the health care field for over a decade. However, her at-home care experience began when, at the age of thirteen, she started caring for her elderly church neighbors as well as her grandmother. This included sleeping at their homes at night, preparing meals and shopping for them. She also had first-hand experience with caring for a loved one when her mother-in-law was diagnosed with dementia. Colleen's area of expertise is working with individuals with dementia and helping both the individual and the family cope, in part by sharing best practices with the families of her clients. Colleen loves working with the seniors in her community. She feels very fortunate to have been very close to her grandparents and aunts & uncles growing up and I believes they were influential in developing her personality and love for helping others. Micah's area of expertise lies in creating and organizing the schedules for caregivers and clients. This includes daily updates on client and employee changes. Micah works to ensuring that all clients are staffed with qualified employees before she goes home for the evening. She prides herself on developing a system of staff communication that ensured proper implementation of treatment plans and comprehensive patient care. Micah received a Bachelor's of Science in Psychology and is working on my MS in Clinical Psychology at Wright Institute. She received her degree in Business Administration, Marketing and previously held a position as an account manager at another home care agency.
