Shelley Nhan

Staffing Manager

Shelley brings strong organizational and communication skills to her role with Home Care Assistance. She prides herself on working well in a fast-paced environment and being a problem-solver. Shelley studied at The University of Vermont, where she did two years of nursing school and then graduated with a bachelor’s degree in communications and a minor in nutrition and food science. She is currently attending graduate school and pursuing a master’s in counseling. Shelley enjoys helping and caring for others and believes there is no better feeling than building a relationship that's ever-lasting and making a positive impact in another person’s life. She looks forward to building long-lasting relationships in her community.

Alexis Montinola

Regional Director

Alexis is an industry leader and has over a decade of experience spanning all aspects of home care. One of Alexis’ major focuses is on lowering hospital readmission rates. She achieves this through partnerships with organizations like Monarch, Hoag Hospital and Mission Hospital.

Alexis thrives when managing an operation during phases of growth. She is passionate about providing the best client experience possible. Alexis strives to navigate her team through difficult situations to promote client satisfaction. Thanks to her management style, her business development team is always improving. As the Regional Director for the Los Angeles, Orange County and Inland Empire areas, Alexis brings a fresh approach to expansion. Her goal is to build a dynamic operation throughout the Greater LA region.

Danny Ortiz

Head of Operations

Danny's area of expertise is in operations and business development; he enjoys both areas of this field. Danny has 6 years of experience in his field and a BA in Public Administration from San Diego State University.
He says he really enjoys making a positive impact in the lives of his customers and their families. Home care is something that is new to most people, so getting to know them personally and understanding how he and his staff can be an extension of their family by providing support is extremely important. Danny finds it is very rewarding to work together as a team to provide solutions that put smiles on our customers faces while keeping them safe and healthy at home.

Jonna Hernandez

Jonna's professional experience is in sales, mentoring, and strategic market development. Her education includes a B.A. in Human Communication Studies from California State University, Fullerton and a M.A. in Counseling from Azusa Pacific University. Jonna started in the health care industry in 2011 as an Admissions and Marketing Director for a skilled nursing facility. She has extensive sales experience in the post-acute sector including skilled nursing facilities, home health, and non-medical home care. Jonna's most recent position was the Area Operations and Sales Manager for a non-medical home care agency for Orange County, South Bay, and Inland Empire regions. Jonna's personal philosophy is to change the outlook of how others view and experience senior healthcare. She is absolutely thrilled to be a part of an innovative team that is truly defining how the world is aging.

Tracy Lara

Staffing Manager

I attended UCLA and graduated with a degree in Psychology and a minor in both Spanish and Applied Developmental Psychology.
For the past four years I have worked closely with preschool children and their families. The past two years I worked and provided ABA therapy to children and adults with Autism. I also worked closely with their families, training them and supporting them on such a difficult journey . One of my biggest passion's in life is helping others in need and providing them with my help, support, and compassion. I'm new to the senior care industry but I have a firm belief that in the same way that I have helped children with special needs, I can help seniors and their families. In the future, I hope that my background in Psychology can allow me conduct research on ways to improve the way seniors age. I have a strong interest in the CTM Program.

Laura has over a decade of experience in the medical field, where she has served as a medical assistant and front office manager. She is a certified MA with the state of California and worked for 10 years with an Internal Medicine DR who specialized in Geriatrics. Elaina brings over 10 years of customer service experience to her role at Home Care Assistance, Her specialty is educating clients on our services with compassion and understanding. Elaina loves working as a client advocate to help each client find the best solution for their needs. Relieving stress from family caregivers and helping seniors to remain in their homes is what she finds most fulfilling in her role. Elaina's favorite part about working in customer service is answering the questions a client may not have even realized they need to ask. Mina is a Licensed Vocational Nurse and began her career in health care in 2011. Since then, she has learned a lot in her many roles. She started as a hospice nurse and then became a nurse for concierge doctor, home health nurse, and intake coordinator for a home health & caregiving agency. Mina believes that helping people who can't help themselves is what we are here for. By working for Home Care Assistance, she feels she can make a difference in the lives of seniors and help the home care industry thrive.

Danielle brings over 20 years of customer service and operations experience to her position at Home Care Assistance. Danielle started in the health care industry in 2014 as a Managed Care Coordinator for a specialty pharmacy. She worked closely with patients and physicians coordinating their pharmacy care plan. Danielle has a genuine passion for helping people and serving the senior community. Yvonne has over ten years of experience in the medical field, working for a large in-home respiratory provider. She worked closely with physicians and hospitals to provide life-sustaining equipment dedicated to keeping aging seniors in their own homes. Yvonne is skilled at working with clients’ loved ones to help ensure an easy transition even though the seniors may have difficult health issues.
