Home Care Assistance Hosting “Hospital to Home” Open House with Leading Experts on Aging and Caregiving
Home Care Assistance CEO Lily Sarafan will attend the event which will cover the country’s hospital readmission crisis at a seminar on the transition for aging adults from the hospital back to their homes
(Washington, DC – March 18, 2016) Next week, Home Care Assistance (HCA), the leading provider of home care for seniors across the United States, Canada and Australia is hosting an open house and seminar on the transition from hospital to home. Home Care Assistance CEO Lily Sarafan, who is speaking at the 2016 “Aging in America Conference” in Washington, D.C. will be attending the open house. The presentation will be led by Joanna Navas, RN, a Nursing Supervisor for Home Care Assistance’s Bethesda location who has extensive expertise on helping patients smoothly transition home after a hospital stay.
One of the leading causes of hospital readmission is the lack of proper support following a hospital discharge. Each year hospitals across the country spend billions of dollars to treat patients readmitted within 30 days of discharge.
