Meet the Team
Angela attended Norfolk State University majoring in Computer Science. After the birth of her children she returned to school and received her AA in medical transcription from Clayton State College and University. Angela worked as an allied health professional for over 18 years as a certified medical transcriptionist before arriving at Home Care Assistance. She has been with Home Care Assistance for over 8 years. Angela has always had a desire to care for others, but that passion grew even greater after the birth of a special needs child as well as caring for her parents. She has devoted herself to ensuring that each and every Home Care Assistance client is matched with a well-qualified and caring caregiver and receives the best quality care possible.
Triff Cook, MBA, CMC is the Director of Client Services at Home Care Assistance. She is also a Geriatric Care Manager. A Geriatric Care Manager (or Aging Life Care Specialist) is a health and human services specialist who helps families who are caring for older relatives. The Geriatric Care Manager is trained and experienced in any of several fields related to care management. Triff is certified in Alzheimer’s Care and is a resource for families of older adults and others with chronic needs, including helping those suffering from Alzheimer’s Disease or Parkinson’s or exhibiting symptoms of dementia. She speaks often to groups on topics related to healthy aging.
Triff is a graduate of Dartmouth College and has an MBA from Emory University. She received her certification in Geriatric Care Management in 2009 from the National Association of Professional Geriatric Care Managers. An Atlantan since 1980, Triff paints, gardens and plays bridge-a game long associated with healthy aging!
Hampton brings over a decade of experience as a Registered Nurse to her position as RN Care Manager. Previously, she worked at Emory University Hospital and specialized in Critical Care Medicine (ICU). During her time at Emory, she served on multiple hospital committees, which were created to avoid common health care related problems that plague the elderly population. This research based knowledge is now passed on to our clients as she helps seniors remain healthy and live safely and independently at home.
An Atlanta native, Pam is a graduate of the University of Georgia and Georgia State University's graduate school. After a 30 year career in the advertising business as a Senior Vice President with J. Walter Thompson, she came to work at Home Care Assistance Atlanta in November 2009 as a case manager. Experience as a hospice volunteer convinced her that the next and most gratifying stage in her career would be in the realm of helping people and their families at the most troubling and confusing stages of their lives. That is what Home Care Assistance has provided. Pam is an avid gardener and supporter of the Atlanta theater community.
Melissa LeBoeuf is a 22-year veteran of Senior Services and Health Care in Atlanta. She received her Nursing Degree from University of North Carolina at Greensboro. Melissa has extensive experience working in assisted living facilities, hospitals, rehabilitation centers, outpatient acute care, hospice, and private duty home care. She currently serves on the board for The American Parkinson’s Association Georgia Chapter. She is also involved with multiple senior network groups throughout metro Atlanta.
In her position as Client Case Manager, Melissa helps foster quality care for her clients as well as maintaining professional relationships in the community and connecting clients to resources to improve their quality of life. The position is a perfect for Melissa, whose passion is working with the aging population and advocating for elders.
Away from work Melissa enjoys quality time with her husband and two children, their golden retriever. She loves travel and watching her children's sports activities, including equestrian events, football and lacrosse.
Annemarie Schwarzkopf has been working in the field of “elder care” for over 12 years. She was a consultant for A Place for Mom for 9 years helping countless families negotiate the maze of senior care options from home care, to senior independent living, to assisted living and into nursing care. This work included meeting with families, social workers, physicians, hospital and nursing home staff as well as senior living communities.
Like many baby boomers, Annemarie’s interest in helping seniors developed from her personal experience caring for her own parents. Home Care Assistance was the agency she used to come into their home and provide care for her parents. Annemarie now concentrates on case management and marketing for Home Care Assistance. She loves the personal contact she has with her clients.
Kat graduated from the University of Georgia. She is relentlessly motivated by her commitment to live with a loving ethic. Kat describes herself as creatively analytical, which helps her to find the best solutions to client's care needs. She is interested in the Home Care field because she has a serving spirit and loves to help people. On a personal note, she has many relatives who are aging, which has inspired her to make lives better through home care.
Arnetta Jordan joined the Home Care Assistance team in 2013. A native of Montgomery, Alabama she comes from the medical community with expertise in Medical Billing and Coding and has experience in working in a medical family practice. She is a Staffing Manager and helps our clients have the best possible experience with their care. She is a passionate advocate of older adults and always has our clients best interest at heart and firmly believes Tomorrow is a New Day!
Susan has been a registered nurse since 1978. Her nursing career includes hospital, thoracic surgeon’s nurse, Minimum Data Set in nursing care facilities and caring for aging parents, friends, and family. Susan joined the HCA Atlanta team in 2009, where she continues to share her knowledge and expertise.

Terry is an experienced home care executive and Certified Senior Advisor, CSA. In his last position, he was the founder and CEO of Angel Heart Homecare located in Marietta, GA. He managed the company from 2009 to the end of 2016 when the agency was sold. Terry was also Vice President of Private Duty Services at Interim Healthcare of Atlanta, where he created a start-up private duty program for the Atlanta Home Health branch and grew the program to over $1,200,000 in annual revenue. He is passionate about the home care industry and helping seniors live healthy, independent lives at home.
Michelle Smith is an RN Supervisor and recent addition to the Home Care Assistance team. Having relocated to Atlanta from New Jersey in 2016, she brings with her over 25 years of experience as an RN. Michelle has worked in a number of settings including hospitals, home health care providers and retirement communities. When not serving clients, Michelle enjoys spending time with her husband and two daughters. She also spends time volunteering with her daughters’ activities as well as local charity organizations.
Vientese studied Counseling and Human Services at Atlanta Christian College (Point University) and has five of experience working in HR and Recruiting in a Homecare setting. She is passionate about recruiting the best caregivers in an effort to provide tailored care to our clients. Her personal philosophy is: Always value your character and integrity, whether it's in your personal life or in the marketplace.
Mina is a Licensed Vocational Nurse and began her career in health care in 2011. Since then, she has learned a lot in her many roles. She started as a hospice nurse and then became a nurse for concierge doctor, home health nurse, and intake coordinator for a home health & caregiving agency. Mina believes that helping people who can't help themselves is what we are here for. By working for Home Care Assistance, she feels she can make a difference in the lives of seniors and help the home care industry thrive.
James brings over 15 years of customer service experience to his position at Home Care Assistance. He prides himself on his ability to provide quality customer service and understand the needs of those he serves. He has a genuine passion for helping people, which is what drew him to the home care industry. He looks forward to making a difference in the lives of seniors and their loved ones every day.
Laura has over a decade of experience in the medical field, where she has served as a medical assistant and front office manager. She is a certified MA with the state of California and worked for 10 years with an Internal Medicine DR who specialized in Geriatrics. Laura enjoys helping families and making them aware of all the options they have regarding home care. She hopes to help seniors live safely and independently at home.
Barbara brings years of experience in the senior care industry to her position as Care Advisor for Home Care Assistance. She previously worked at A Place for Mom, where she received an in-depth education on all of the options in senior care. It became clear to her early in her career that almost everyone wants to stay independent for as long as possible and that there is a deep desire to remain in one's own home. She also saw this to be true in her own personal experience with her mom and both of her in-laws. While the home care was for her loved ones, she also saw how the experience directly benefited her own life. She believes that having a trusted homecare partner made it possible for her to be a daughter again. Barbara came to Home Care Assistance with a passion for helping others live out their lives with happiness and dignity in the home that holds their memories. She feels she has truly done her job when she can assist families in finding the peace of mind that she was able to have during her time of need.
Nicole began her health care journey in 1995 and brings together everything she has learned in her over 20 years of experience to her role as Staffing Manager for Home Care Assistance. Nicole has 10 years’ experience managing long-term care solutions for seniors, 5 years’ experience in the home health field in addition to 8 years’ experience in ICU and CCU. Furthermore, she is a trained CNA and EMT. Nicole loves being a Staffing Manager because it is a puzzle to find the perfect Caregivers for a family, and she enjoys making the pieces of that puzzle fit just right! Her personal philosophy is that teamwork makes the dream work.
